I work closely with healthcare providers to develop and implement care plans, coordinate appointments and services, and serve as a resource for patients and their families.I have strong communication and organizational skills that are essential for success in this role which has been proven effective in decreasing hospital re-admission/ ED visits to improve client health outcomes.
