Nurse - Home Health Clinical Documentation and QA Specialist

Work Experiences:
* USA Quality Assurance and Verification for Therapy Notes (OT, ST and PT) OASIS and other documentation using Wellsky/Kinnser, Pariox, Point of Care, Axxess, Devero and Kantime EHR;
communicate with clinicians and partner agencies about issues on notes using Slack, Voice, Ring Central and EHR

* USA Home Health Case Manager / Clinical Documentation Specialist (Medicare, Medicaid and Private Insurance)
– Responsible for completing Skilled Nursing (SN) and HHA notes, PT notes, OASIS for Start of Care, Resumption of Care, Transfer, DC Transfer, Recertification and Discharge, Non-OASIS Assessments, Plan of Care (POC)/485 creation, OASIS coding, creation of Physician's order, Communication Note, Discharge Summary

Patient's Calendar / Chart Management / Scheduling:
Plotting of visits (HHA, PT and SN) to Electronic Health Record: HHExchange, Synergy, Wellsky/Kinnser and Data Soft Logic
Medicine Reconciliation, administer Doctor's Orders and plotting them to EHR, admission assessment,referrals to other healthcare companies such as PT/OT/ST, DME suppliers, pharmacy and laboratory, referrals / coordination thru emails, fax (Phonefusion, Metro fax, Ring Central) and phone calls to other healthcare agencies and healthcare givers such as MSW, HHA and Skilled Nurses, communication with patients, follow up of HNP and medical records to health care agencies and other related tasks.

Chart Audits & Compliance:
Conducts routine chart audits for SOC, Recertification, ROC, Transfers, and Discharges. Ensure all required documents are present (physician orders, consents, supplemental orders, POCs). Monitor visit frequency, discipline coordination, and regulatory timelines. Identify deficiencies and provide corrective action guidance.

* Registered Social Worker: 8 years of experience working with Philippine government agencies. (2 years as Social Welfare Officer in MSWDO and 6 years as Project Development Officer in DSWD)

* Healthcare and NGO Experience: 4 years as a Staff Nurse in various settings, including a polyclinic, orphanage, medical missions, and as an Administrative Assistant for a Non-Governmental Organization (NGO).

*Teaching Experience: 4 years as a part-time online instructor, teaching Research and Social Work major subjects.
